The Active Recovery Clinic is a multidisciplinary rehabilitation facility specializing in the assessment and treatment of injured people. Our goal is to return clients to their pre injured lifestyle at work, home and sports.

The Active Recovery Clinic is a private facility and is not covered under OHIP. Our services are covered through extended health benefits, workers compensation as well as auto insurance. You should contact your insurer to determine any limitations or requirements for the claim process.

On your first visit, you will undergo a detailed assessment by your therapist. Your evaluation may take up to an hour and will include commencement of treatment as well as education on your injury. Please plan to arrive ten to fifteen minutes prior to your scheduled appointment in order to fill out the necessary paper work. You can down load this from our web site as well.

Please wear appropriate clothing for your assessment. If your injury is your neck or shoulder, a tank top would work best. If it is low back or lower extremity, then shorts are most appropriate.

Informed consent is required by law by the clients. Patients may stop assessments or treatment at anytime without reason, as well, they may request another therapist if they feel necessary. Children must be accompanied by an adult.

The most beneficial attendance schedule for you at the clinic will be determined by your therapist and yourself. Consistency of attendance is very important in ensuring a successful recovery. It is not unusual for patients to attend treatment 2 - 3 times per week for 4 - 8 weeks for common orthopedic conditions. Your visits are scheduled specifically for you. If you are unable to make your appointment, 24 hours notice is required or a fee will be applied.

WSIB claimants are required to provide SIN#, OHIP#, WSIB claim #, date of accident, date of birth as well as employer name and address, and what your work status is(still working or off due to injury)

Motor Vehicle Accident Claimants are required to complete an Ontario Claim Form 1-59 Application for Benefits. This should be provided by your auto insurer. Please note that by law, your private extended health insurance must be exhausted before the auto insurance provider will pay for rehab treatment. If you have any questions regarding the process, do not hesitate to ask us.
